We contract with many insurance plans including Medicare to provide medical care to their members. When you request an appointment, please verify that our providers are on your plan. If so, we will file your insurance claim for you. You will be responsible for your plan’s office visit co-pay and any deductible at the time of your appointment. Upon arrival to the office, please present your health insurance card and driver’s license.
